After you have collected the items for the kit and the $100 contribution:

• Pack the items carefully in a sturdy carton for shipment to I.M.A. Secure the box with reinforced tape.
• Please be sure that your church's name and address are clearly marked as the "sender" on the outside of the box.

For local church and annual conference credit, give your check to your local church treasurer, who will forward it to your annual conference treasurer. Gifts may also be forwarded to:

Indicate the date the HFK was shipped, how it was shipped, and that the HFK is to be designated for The United Methodist Church.
